**Primary Care Diabetes Specialist**

We are seeking a qualified nurse to join our nationwide diabetes team as a primary care diabetes specialist.

As a primary care diabetes specialist, you will be responsible for identifying patients with sub-optimally controlled Type 2 diabetes, optimising their oral therapy, and referring for specialist insulin treatment when necessary.

You will play a pro-active role in improving patient outcomes, developing your skills and expertise, and working collaboratively with other healthcare professionals.

Requirements
  • A Diploma in Diabetes or equivalent;
  • At least three years of experience managing diabetes in primary care;
  • Knowledgeable and confident in providing education, support, and advice to patients with Type 2 Diabetes;
  • A full driving licence and current NMC registration.

We offer a competitive salary of £58,000 - £72,000 per annum, depending on experience and location, plus additional benefits including:

  • A minimum of 25 days annual leave, plus bank holidays (increasing after five years' service) - pro rata for part-time staff;
  • An annual bonus incentive scheme worth up to 10% of annual salary;
  • A car allowance or company car, depending on contract, plus a fuel card for business miles;
  • Company pension, including salary sacrifice;
