Job Summary
A Type 2 Diabetes Care Advisor is required to join our nationwide team, working in primary care across the country. You will work in a pro-active manner, identifying sub-optimally controlled patients with Type 2 diabetes, and providing them with the best possible care. The role involves optimising oral therapy, conducting point of care testing, and referring for specialist insulin treatment where necessary.

Key Responsibilities
* Work in primary care across the country* Identify sub-optimally controlled patients with Type 2 diabetes* Optimise oral therapy* Conduct point of care testing* Refer for specialist insulin treatment where necessary* Deliver high-quality care to improve patient outcomes* Work in a pro-active manner* Corporate nursing responsibilities

What We Offer
An annual salary of £45,000, plus a range of benefits including a bonus incentive scheme, car allowance or company car, company pension, private medical insurance, NMC/RCN fees, monthly broadband contribution, daily lunch allowance when in practice, and 25 days annual leave, plus bank holidays.
