Diabetes Care Specialist

In line with local and national Diabetes guidelines (NG28), we are seeking a Nurse Practitioner for Chronic Conditions to deliver the highest quality care to improve patient outcomes across various locations in the UK. As a key member of our nationwide team, you will work proactively to identify sub-optimally controlled patients with Type 2 diabetes, optimising their oral therapy and referring for specialist insulin treatment when necessary.
Key Responsibilities:
- Identify sub-optimally controlled patients with Type 2 diabetes
- Optimise oral therapy and conduct point of care testing
- Refer patients for specialist insulin treatment when necessary
Requirements:
A registered Nurse with a passion for delivering high-quality care, you will have excellent communication skills, strong clinical knowledge, and experience working in primary care. You will also be committed to professional development and staying up-to-date with the latest Diabetes guidelines.
Salary & Benefits:
The annual salary is around £45,000 + package, including an annual bonus incentive scheme worth up to 10% of your annual salary, car allowance or company car, company pension, private medical insurance, NMC/RCN fees, monthly broadband contribution, daily lunch allowance, and 25 days annual leave plus bank holidays.
