Job Overview

We are seeking a highly skilled and motivated Mental Health Practitioner to join our Barrow Home Treatment Team (HTT) in Lancashire & South Cumbria NHS Foundation Trust.

The successful candidate will be responsible for providing comprehensive assessments and triage services, as well as signposting or facilitating onward referrals to mental health pathways. The role requires excellent clinical skills, a good working knowledge of mental health and community services, and the ability to work collaboratively with multidisciplinary teams.

Key Responsibilities:

  • Provide thorough assessments and clear onward facilitation of service users to the most appropriate care following assessment.
  • Evidence-based mental health assessments and/or signposting to alternative service provision for individuals requiring a specialist urgent mental health assessment.
  • Follow-up appointments may be offered within the agreed timescale for intervention as part of the ongoing assessment process.

About You:

  • Registered Mental Health Nurse/Social Worker/Occupational Therapist qualification required.
  • Able to understand the difference between crisis planning and contingency planning and to formulate explicit planning in these areas.
  • Knowledge, understanding, and experience of care pathways for psychosis and non-psychosis and relevant interventions.
