About This Role

We're seeking a Support Worker to play a vital role in supporting individuals to live independently, manage their mental health, and achieve their personal goals.

Main Responsibilities:
  • Empowerment through Support: Collaborate with clients to develop self-care routines, build resilience, and manage mental health effectively.
  • Social Inclusion and Connection: Facilitate community involvement and social opportunities to reduce isolation and foster meaningful relationships.
  • Care Planning and Collaboration: Work alongside healthcare professionals to create and implement personalized care plans.

We're looking for compassionate, empathetic individuals with a genuine interest in mental health and wellbeing.

What We Offer:
