About the Role

This is an exciting opportunity to join our Peripatetic Insulin Team as a Community Nurse Practitioner.

As a key member of our team, you will be working closely with patients, families, and other healthcare professionals to provide high-quality, patient-centered care.

The successful candidate will have the skills, knowledge, and experience to assess, plan, implement, and evaluate all aspects of patient care.

You will be responsible for developing and delivering individualized care plans, modifying them as required, and utilizing clinical reasoning skills.

We are looking for a highly motivated, organized, and compassionate individual to join our team.

Key Responsibilities
  • Assess patients' holistic needs and develop individualized care plans.
  • Prescribe and deliver insulin, planned and unplanned, to achieve quality of life and independence.
  • Work within the integrated team to facilitate early discharge from hospital and prevent unnecessary admission.
  • Liaise with GPs, Social Services, and other healthcare professionals to ensure identified needs are met and care coordinated appropriately.
  • Participate in multidisciplinary meetings and contribute to the requisitioning of supplies and equipment.
