About the Role

We are seeking a highly skilled and motivated Peripatetic Insulin Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a key member of our community nursing team, you will be responsible for providing high-quality care to patients with diabetes, working closely with GPs, Social Services, and other healthcare professionals to ensure seamless care coordination.

Key Responsibilities

Assess, plan, implement, and evaluate patient care, developing individualized care plans using clinical reasoning skills.

Devise evidence-based care programs for patients, modifying them as required, and communicate complex information to patients and carers.

Use tact and persuasive skills to gain patient cooperation in care management plans, and liaise with healthcare professionals to ensure identified needs are met.

Participate in multi-disciplinary meetings, ensure own caseload is planned and prioritized, and contribute to requisitioning supplies and equipment.

Requirements

Must hold a full and valid driving license, have access to a vehicle, and hold a Diploma or 1st-level degree, RN qualification, and current NMC registration.

Experience of working within a clinical setting, preferably in a community setting, and highly motivated with good observational and reporting skills, time management skills, and ability to work flexibly.
