About the Role

We are seeking a highly motivated and experienced Peripatetic Insulin Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a key member of our community nursing team, you will be responsible for providing high-quality care to patients with diabetes, working closely with GPs, social services, and other healthcare professionals to ensure seamless care coordination.

Key Responsibilities
  • Assess, plan, implement, and evaluate patient care, developing individualized care plans using clinical reasoning skills.
  • Devise evidence-based care programs for patients, modifying them as needed.
  • Communicate complex information to patients and carers, adapting explanations to meet individual needs.
  • Liaise with healthcare professionals and agencies to ensure identified needs are met and care is coordinated.
  • Participate in multi-disciplinary meetings, contributing to patient care and service development.
  • Ensure patient records are accurate and up-to-date, using SystmOne for data input.
  • Work flexibly to accommodate patient and service needs, using own initiative and good time management skills.
Requirements
  • Current RN qualification and NMC registration.
  • ENB 998/mentorship preparation or equivalent.
  • Experience of working in a clinical setting, preferably in community nursing.
  • Good observational and reporting skills, with ability to concentrate during patient care and data input.
  • Basic IT and keyboard skills, with ability to use assistive technology.
  • Able to kneel, bend, and stoop, with ability to manoeuvre limbs and patients using handling aids.
