About the Role

We are seeking a highly motivated and experienced Peripatetic Healthcare Assistant to join our team at Norfolk Community Health and Care NHS Trust. The successful candidate will work closely with our Unplanned Care Response Team to provide high-quality care to patients in the community.

Key Responsibilities

The post holder will be responsible for:

  • Working within the integrated team to facilitate early discharge from hospital and prevent unnecessary admission to hospital
  • Providing a seamless, integrated service to our patients by working with all healthcare professionals and statutory/non-statutory agencies
  • Supporting patients with re-enablement following hospital admission or new diagnosis, wherever possible
  • Working with the Band 5 Peripatetic Insulin Nurse to ensure caseload is managed effectively
  • Supporting the West Unplanned Care Response Team with patient visits, such as urgent INRs, ecgs, wound care, and double-ups
  • Working closely with Triage and Diabetes Specialist Team to ensure new patient discharges are reviewed in the community

Person Specification

The ideal candidate will have:

  • NVQ 2 Health and Social Care or equivalent knowledge and experience
  • Experience of working within a clinical setting
  • Good observational and reporting skills
  • Ability to work flexibly to accommodate patient/service needs
  • Ability to communicate effectively using the English language in both verbal and written forms
  • A full and valid driving licence and access to a vehicle
