About the Role

We are seeking a highly motivated and experienced Peripatetic Insulin Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a Peripatetic Insulin Nurse, you will be responsible for assessing, prescribing, and delivering insulin to patients in their own homes, care homes, and clinics.

Key Responsibilities

Assess patients' holistic needs and develop individualized care plans

Deliver high-quality patient care, including insulin administration and education

Work closely with GPs, Social Services, and other healthcare professionals to ensure seamless care coordination

Participate in multi-disciplinary meetings and contribute to the development of care plans

Requirements

Registered Nurse with current NMC registration

Experience of working in a clinical setting, preferably in a community setting

Strong communication and interpersonal skills

Ability to work flexibly and manage a caseload
