About the Role:
This is an exciting opportunity to work in a fast-paced community nursing environment, where you will be responsible for delivering high-quality insulin care to patients in their own homes.
As a Mobile Insulin Care Specialist, you will work closely with our multidisciplinary team to assess, plan, implement, and evaluate patient care, ensuring that each individual receives tailored support to manage their diabetes effectively.
You will have excellent communication skills, with the ability to communicate complex information clearly and sensitively to patients and their families.
Main Responsibilities:
- Assess patient needs and develop personalized care plans in collaboration with healthcare professionals
- Deliver high-quality insulin care to patients in their own homes, including administering medication, monitoring blood glucose levels, and providing education on self-management techniques
- Work closely with our multidisciplinary team to coordinate care, share knowledge, and ensure seamless transitions between healthcare services
- Provide emotional support and guidance to patients and their families, promoting independence and empowering them to manage their condition effectively
- Contribute to the development of best practices and guidelines for insulin care within the trust, sharing your expertise and experiences with colleagues
Requirements:
- Registered Nurse qualification (RGN or RPN) and current NMC registration
- Experience working in a community nursing setting, preferably with a focus on diabetes care
- Strong communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare professionals
- Ability to work independently, prioritize tasks, and manage time effectively in a fast-paced environment
- Basic IT skills, with familiarity with electronic patient records and other healthcare software
