Peripatetic Insulin Specialist

About the Role
We are seeking a highly motivated and experienced Peripatetic Insulin Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a key member of our community nursing team, you will be responsible for providing high-quality care to patients with diabetes, working closely with GPs, Social Services, and other healthcare professionals to ensure seamless care coordination.
Key Responsibilities
- Assess, plan, implement, and evaluate patient care, developing individualized care plans using clinical reasoning skills.
- Devise evidence-based care programs for patients, modifying them as required, and communicating complex information to patients and carers.
- Liaise with healthcare professionals to ensure identified needs are met and care is coordinated appropriately.
- Participate in multi-disciplinary meetings, Gold Standard Framework, and contribute to the requisitioning of supplies and equipment.
- Ensure informed consent is obtained prior to initiating interventions and adhere to infection control policies.
Requirements
- Must hold a full and valid driving licence and have access to a vehicle.
- Diploma or 1st level degree, RN Qualification, Current NMC registration, and ENB 998/mentorship preparation or equivalent.
- Experience of working within a clinical setting, preferably in a community setting.
- Highly motivated, able to provide high standards of care, and possess good observational and reporting skills.
