About the RoleThis exciting opportunity has arisen for a skilled Care Coordinator Specialist to join our team in Surrey Heath, supporting the delivery of high-quality patient care.
We are seeking an experienced individual to work closely with healthcare professionals and patients to coordinate access to services, provide advice, and ensure timely and efficient health and care planning.
The successful candidate will have excellent communication skills and be able to travel locally as required. A creative and flexible approach to working with people with diverse support needs is essential.
Main Responsibilities- Coordinate access to healthcare services, including hospital appointments and referrals, ensuring timely and efficient patient care.
- Work closely with healthcare professionals and patients to develop personalized care plans, taking into account their unique needs and goals.
- Provide education and support to patients on disease prevention and management, promoting healthy lifestyle choices and empowering them to take control of their health.
- Collaborate with healthcare teams to ensure seamless transitions of care, reducing hospital readmissions and improving patient outcomes.
We are Surrey Heath Community Providers Limited, a GP Federation that represents member practices and champions primary care by working with local general practice and system partners in the provision of community-based healthcare services.
