Care and Support Specialist

**About the Role**
The Proactive Care Practitioner will provide a responsive, safe, effective caring service to support individuals living with mild to moderate frailty and long-term conditions. They will work closely with the Integrated Care Team, Frailty MDT, and local GP teams to provide proactive care services for patients with frailty or those with a long term condition identified as requiring an anticipatory approach.
**Key Responsibilities**
To devise effective personalised care plans for each patient, recognising their individual needs and circumstances. This includes working closely with multidisciplinary teams to ensure comprehensive and coordinated care delivery.

We are a federation of 7 GP practices across 10 sites, covering a population of over 103,000 patients in Surrey Heath. Our goal is to provide safe and compassionate care to our patients through community-based healthcare services.
**Person Specification:**
* Maths and English GCSE's (C and above) or equivalent.
* Registered Healthcare Professional, with either the NMC or HCPC registration.
* Masters degree or equivalent experience gained through on-going personal development and training.
**Requirements:**
* Car driver with valid licence.
* Reliable and flexible, with a commitment to attend forms or training as learning needs are identified.
**Estimated Salary:** £40,750 per annum.
