**Job Title:** Advanced Long Term Condition Nurse Specialist
**Job Summary:** We are seeking an experienced nurse specialist to join our team at Surrey Heath Community Providers Limited. As a key member of our multidisciplinary team, you will provide high-quality nursing care and management to patients with complex health needs.
**About Us**
Surrey Heath Community Providers Limited is a federation of 7 GP practices across 10 sites, covering a population of over 103,000 patients. We pride ourselves on providing safe and compassionate care to our patients across our range of primary care and unplanned healthcare services.
**Responsibilities**
- Deliver high-quality nursing care and management to patients with complex health needs
- Undertake holistic assessments, make nursing diagnoses, and develop plans of care
- Provide education and support to patients and their families
- Collaborate with other healthcare professionals to ensure comprehensive care
**Requirements**
- Degree in Nursing or equivalent
- Post-registration study in relevant area of practice/long term condition(s)
- NMC registration - Adult/General Nurse
- Experience of caseload management and triaging referrals
**Salary:** **£42,000 - £55,000 per annum** (dependent on experience)
