We are seeking a dedicated and compassionate Care Coordinator to join our team at Elysium Healthcare. As a Care Coordinator, you will play a vital role in providing high-quality care and support to individuals with learning disabilities and autism.

Key Responsibilities

  • Conduct ward rounds to assess the needs of service users and develop personalized care plans.
  • Analyse behaviours and adapt care accordingly with colleagues to ensure the best possible outcomes.
  • Update patient records to reflect changes in service user needs and progress.
  • Provide overall care and support, which may include assisting with personal hygiene.
