About the Role

We are seeking a skilled and compassionate Care Coordinator to join our team at Elysium Healthcare. As a Care Coordinator, you will play a vital role in providing high-quality care and support to individuals with Learning Disability and Autism conditions.

Your day-to-day will involve working closely with colleagues to analyse behaviours, adapt care plans, and provide overall support to our service users. This may include assisting with personal hygiene, updating patient records, and participating in ward rounds.
