About Us

We are seeking a highly motivated and skilled Skin Cancer Care Coordinator to join our team at Sheffield Teaching Hospitals NHS Foundation Trust.

The successful candidate will be responsible for providing comprehensive support and information to patients and families affected by skin cancer, ensuring seamless care as they move along their cancer pathway and across provider organisations.

The role will involve regular liaison between clinical and administrative team members and acting as a conduit between patients and healthcare professionals.

The ideal candidate will have relevant experience in managing small projects, navigating referral pathways, and working in a busy environment that requires flexibility.

Responsibilities

  • Assist the skin CNS Teams within the daily management and support of people affected by skin cancers regionally and nationally.
  • Provide an additional source of contact for patients and their families/carers, helping to ensure seamless care as they move along their cancer pathway and across provider organisations.
  • Coordinate and navigate supportive care, signposting patients towards specialist resources to enable them to maximise the quality of care and support they receive.
  • Work closely with the Clinical Nurse Specialists/MDT members and the wider team to ensure a quality service is delivered to those people affected by cancer.

Requirements

  • Relevant Health or social care experience at Band 3 or equivalent, or transferable knowledge & experience of working with the public.
  • Understanding of key referral pathways to Allied Health Professionals to support plans of care.
  • Experience in managing small projects and navigating referral pathways.
  • Previous Involvement with Data collection systems.
  • Experience of dealing with challenging situations.

Desirable

  • Experience in palliative care, cancer care, haematology, or oncology environments.
  • Previous experience of supporting patients and families affected by cancer.
  • Previous experience of participation in audit.
