Job Summary

We are seeking an experienced Patient Support and Navigation Coordinator to join our Skin Cancer Care Team. This is an exciting opportunity for a highly motivated individual to make a real impact on the lives of our patients and their families. The successful candidate will provide comprehensive support and information to patients affected by skin cancer, working closely with our clinical teams to ensure seamless care throughout their treatment journey.

About the Role

The Patient Support and Navigation Coordinator will be responsible for providing expert navigation and coordination of supportive care for patients with skin cancer. This will involve working closely with our Clinical Nurse Specialists, multidisciplinary teams, and other healthcare professionals to ensure that patients receive the best possible care and support. The role requires excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities
  • To assist the skin CNS Teams within the daily management and support of people affected by skin cancers regionally and nationally who are receiving treatment and care at the Royal Hallamshire and Weston Park Hospital.
  • To provide comprehensive support and information to patients and families affected by cancer, including navigation and coordination of supportive care.
  • To work closely with Clinical Nurse Specialists, multidisciplinary teams, and other healthcare professionals to ensure seamless care throughout the treatment journey.
  • To communicate effectively with patients, families, and healthcare professionals, ensuring that all parties are informed and engaged in the patient's care plan.
Requirements

To be considered for this role, you will need:

  • Relevant health or social care experience at Band 3 or equivalent, or transferable knowledge and experience of working with the public.
  • Understanding of key referral pathways to Allied Health Professionals to support plans of care.
  • Experience in managing small projects.
  • Previous involvement with data collection systems.
  • Experience of dealing with challenging situations.
  • Previous experience of working within a multidisciplinary team, or equivalent.
  • Experience of working in a busy environment that requires flexibility.

  • A salary of £25,587 - £30,451 per annum, depending on experience.
