Skin Cancer Care Coordinator

About the Role
We are seeking a highly motivated and organised individual to join our Skin Cancer Care Coordinator team as a Patient Support Specialist. The successful candidate will provide comprehensive support and information to patients and families affected by skin cancer, working closely with our Clinical Nurse Specialists and multidisciplinary team.
Key Responsibilities
- Assist in the daily management and support of people affected by skin cancers regionally and nationally.
- Provide an additional source of contact for patients and their families/carers, ensuring seamless care as they move along their cancer pathway.
- Work closely with clinical and administrative team members to provide comprehensive support and information to patients and families.
- Act as a conduit between patients and healthcare professionals, facilitating communication and coordination of care.
Requirements
- Relevant health or social care experience at Band 3 or equivalent.
- Understanding of key referral pathways to Allied Health Professionals to support plans of care.
- Experience in managing small projects and dealing with challenging situations.
- Previous involvement with data collection systems and experience of working in a busy environment.
