Care Coordinator Role Summary
We are seeking a skilled Care Coordinator to join our team at Brook Street Social Care NMR. The successful candidate will be responsible for effectively coordinating care delivery for clients, staff, and the business.
Key Responsibilities:
- To schedule Home Care rotas, ensuring client calls are allocated as close to their requirements as possible.
- To allocate care calls to staff, maximizing efficiency and considering travel time, breaks, and availability.
- To ensure care calls are allocated in accordance with relevant legislation, including Working Time Regulations.
- To work with the Senior Care Coordinator to facilitate new clients, where possible.
- To ensure client preferences are met by providing the appropriate Care Support Worker, wherever possible.
- To work with the Senior Care Coordinator and Care Supervisor to update and maintain client files, including preferences, changes to requirements, respite, holidays, service suspensions, and cancellations.
- To keep the Senior Care Coordinator updated of any changes in client requirements, ensuring scheduled care calls are invoiced appropriately.
- To be on call for emergencies, out of hours service, Monday through to Sunday on a rotational basis.
- To undertake client visits when calls are not covered.
Requirements:
- Compassionate and enthusiastic about care provision and the Health and Social Care sector.
- Experience of using MS Word and Outlook.
- Full driving licence and access to own vehicle.
Salary and Benefits:
£24,150 per annum, increasing to £26,000 after successfully passing probation. A full enhanced DBS check is required, which will be paid for by the company. You will also be provided with your ID badge and all other equipment at no cost to you.
