Job Summary

We are seeking a highly skilled and experienced Lead Community Health Specialist to join our team at Kingston and Richmond NHS Foundation Trust. As a key member of our community healthcare team, you will play a crucial role in delivering high-quality patient care and support to individuals in their own homes.

About the Role

The successful candidate will be responsible for supporting the Locality Nursing Team Manager in the delivery of the locality nursing service. This will involve providing line management and leadership for the community nursing team, ensuring that patients receive holistic assessments and tailored care plans. You will also take a lead on specific areas of nursing practice, mentoring less experienced nurses and promoting best practice knowledge and expertise.

Person Specification
  • A qualified RGN (Registered General Nurse - Adult) with current registration with NMC - Part 1
  • Relevant post-registration study at diploma/degree level and relevant CPD e.g. palliative care, tissue viability
  • Diploma or BSc (Hons) Community Health (DN)

The salary for this position is £42,939-£50,697 per annum.
