Community Healthcare Specialist

South Tees Hospitals NHS Foundation Trust is seeking an experienced Nurse to join their Community Nursing Team as a Staff Nurse within the Hambleton & Richmondshire locality.
About the RoleThis role will provide high-quality, holistic care for people in their own homes and residential settings, requiring a variety of skills including wound care, continence management, palliative care, health promotion, venepuncture, and more. The successful candidate needs to be prepared to adapt to the changing needs of the Trust and the community models of care as they arise.
Main Responsibilities- Provide clinical care, innovative practice, and support to the District Nurse Sister and Clinical Lead to our ever-developing teams.
- Work closely with GP practices and other partners, supporting patients with highly complex and often high acuity needs to prevent unnecessary hospital admission.
The ideal candidate will have:
- A Diploma or Degree in Adult Nursing.
- Experience of Multi-disciplinary working.
- Commitment to evidence-based practice.
South Tees Hospitals NHS Foundation Trust offers leadership and improvement training to all new staff. This training programme has been designed to support our staff in developing their understanding of leadership and management skills. You will be expected to attend the following training sessions.
