About the Role:

This is an exciting opportunity to join Doylestown Health as a Patient Care Coordinator. In this role, you will be responsible for coordinating optimal transitions of care and supporting high-quality patient outcomes.

Key Responsibilities:

  • Develop and implement strategies to improve patient satisfaction and reduce readmissions.
  • Collaborate with healthcare providers to ensure seamless transitions of care.
  • Maintain accurate and up-to-date patient records.

Requirements:

  • Education: High school diploma, medical assistant diploma from an accredited vocational institution, or a community college course in medical assisting.
  • Experience: Minimum 3 years of recent experience working in a medical facility as a medical assistant preferred.
  • Skills: Strong analytical and problem-solving skills, ability to work effectively in a fast-paced environment.

Estimated Salary: $75,000 - $90,000 per year
