About the Role


We are seeking a highly skilled and dedicated Healthcare Assistant to join our Rapid Response & Intermediate Care Team in Buckinghamshire.


Key Responsibilities



  • Provide nursing, social, and rehabilitative care to patients in their own homes, working collaboratively with the multidisciplinary team.
  • Deliver defined care plans, including nursing, physiotherapy, and occupational therapy-based interventions, to support patient recovery and independence.
  • Conduct assessments and develop care plans, in consultation with patients, their families, and other healthcare professionals.
  • Work closely with the Rapid Response & Intermediate Care Team to ensure seamless care coordination and timely discharge.
  • Provide education and support to patients and their families on disease management, self-care, and healthy living.
  • Participate in quality improvement initiatives and contribute to the development of best practices within the team.

Requirements



  • Previous experience in healthcare, preferably in a community or acute setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team, with a flexible approach to meet the changing needs of patients and the service.
  • Valid driving license and access to a car for work purposes.
  • Good understanding of Integrated Team working and the ability to demonstrate a patient-centered approach.
  • Ability to maintain confidentiality and adhere to NHS policies and procedures.
