About the Role

We are seeking a skilled and compassionate Healthcare Assistant to join our Rapid Response & Intermediate Care Team in Buckingham and surrounding villages. As a member of our team, you will provide nursing, social, and rehabilitative care to designated patients, working collaboratively with senior staff to deliver high-quality care.

Main Responsibilities

  • Providing general and specific care to adults within competencies
  • Being an integral member of the Rapid Response & Intermediate Care (RRIC) team, providing defined delivery of care including nursing, social, and therapy-based rehabilitative care to patients in liaison with senior staff
  • Taking responsibility for designated elements of the patient treatment protocols and delivery of such treatments in patients' own homes
  • Being responsible for the completion and potential delegation of non-clinical tasks within the team
  • Improving the care and support of patients in the community, so patients can remain safely in their own home for as long as possible
  • Having an innovative approach, excellent communication skills, and willingness to work with other members of the community teams to ensure high-quality service delivery
  • Willingness to further develop your clinical expertise in a range of areas
