Healthcare Assistant Practitioner

Job Summary
We are seeking a skilled and compassionate Healthcare Assistant Practitioner to join our team at Buckinghamshire Healthcare NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and supporting our patients to achieve their goals.
Main Responsibilities
- Deliver clinical and therapeutic care to patients in a clinical setting, working within established protocols and guidelines.
- Conduct patient assessments, develop and implement individual care plans, and evaluate patient progress.
- Carry a caseload of non-complex patients and provide support and guidance to patients and their families.
- Coordinate and undertake safe patient admissions, transfers, and discharges, ensuring compliance with Trust policies and procedures.
- Assist patients with personal care needs, including oral hygiene, grooming, and dressing, while respecting their cultural and religious backgrounds.
- Use equipment and mobility aids safely and competently, and report any equipment failures or accidents to the relevant teams.
