About the Role


We are seeking a skilled and dedicated Healthcare Assistant Practitioner to join our multidisciplinary team in a rehabilitation-focused environment. The successful candidate will work alongside physiotherapists, occupational therapists, nurses, and paramedics to deliver high-quality patient care and support.


Key Responsibilities



  • Deliver clinical and therapeutic care within set protocols and in line with the Assistant Practitioner scope.
  • Perform patient assessments, plan, and deliver high standards of care, developing and evaluating individual care/treatment plans.
  • Carry a caseload of non-complex patients and coordinate safe admissions, transfers, and discharges in accordance with Trust policies.
  • Assist patients in maintaining personal hygiene, enabling and assisting as directed, while respecting cultural and religious needs.
  • Use equipment and mobility aids safely and competently, promptly reporting any failures or accidents.
