Community Healthcare Assistant Practitioner Role

We are seeking a dedicated Community Healthcare Assistant Practitioner to join our team at Buckinghamshire Healthcare NHS Trust. This is an exciting opportunity for the right candidate to make a real difference in the lives of patients and their families.
The role involves working as part of a multidisciplinary team, providing high-quality care and support to patients with complex needs. As a Community Healthcare Assistant Practitioner, you will work closely with physiotherapists, occupational therapists, nurses, and paramedics to deliver excellent patient outcomes.
Key responsibilities include:
- Delivering clinical and therapeutic care within set protocols and guidelines
- Performing patient assessments, planning, and delivering high standards of care
- Developing, implementing, and evaluating individual care/treatment plans
- Coordinating and undertaking safe admissions, transfers, and discharges of patients
- Assisting patients with personal hygiene, grooming, and dressing needs, ensuring dignity and respect at all times
In return for your expertise and dedication, we offer a competitive salary of £28,000 - £34,000 per annum, depending on experience. As a valued member of our team, you will also enjoy:
