About this role

As a member of the Rapid Response & Intermediate Care Team, you will play a vital role in providing nursing, social, and rehabilitative care to patients in their own homes. Your primary objective will be to support patients in maintaining their independence and preventing unnecessary hospital admissions.

Main responsibilities

  • Deliver high-quality care to patients, working collaboratively with the multidisciplinary team to develop and implement personalized care plans.
  • Conduct comprehensive assessments of patients' needs, identifying areas for improvement and implementing interventions to achieve optimal health outcomes.
  • Develop and maintain strong relationships with patients, families, and carers, providing emotional support and guidance throughout their care journey.
  • Work closely with healthcare professionals to ensure seamless transitions of care, promoting continuity and reducing hospital readmissions.
  • Participate in regular team meetings and contribute to the development of care plans, ensuring that patient needs are met and goals are achieved.


Requirements

Essential requirements include previous experience in a healthcare setting, good communication and interpersonal skills, and the ability to work effectively in a team environment. A valid driving license and car are also required for travel to various locations throughout Buckinghamshire.
