About the Role

We are seeking an experienced and enthusiastic RNLD or suitably qualified nurse to join our team as a Clinical Team Lead. As part of our Community Learning Disability Intensive Support Team (IST), you will play a vital role in supporting adults with learning disabilities who may be at risk of placement breakdown.

You will work closely with your IST multidisciplinary team (MDT) colleagues to support a defined caseload, utilizing flexible and creative approaches to achieve positive life outcomes for service users.

This is an exciting opportunity to develop your skills and expertise in providing high-quality care and support to individuals with complex needs.

We offer a competitive salary range of £43,141 - £60,461 per annum, depending on experience.

Key Responsibilities:

  • Co-ordinate assessment and interventions throughout the IST clinical pathway with MDT colleagues, service users, parents, and/or care providers.
  • Develop and implement person-centred care plans that promote positive life outcomes for service users.
  • Collaborate with other teams and services to promote shared formulation and risk management.

Requirements:

  • Professional qualification - RNLD or suitably experienced nurse.
  • Current professional registration with the NMC.
  • Evidence of continuing professional development and maintenance of portfolio.
  • Qualification in supervision or mentoring.
  • Evidence of relevant training or currently undertaking e.g. Positive Behaviour Support, Applied Behaviour Analysis.
