This exciting opportunity as a Clinical Lead within the Midlands Partnership NHS Foundation Trust's Community Learning Disability Team is your chance to join a passionate team of professionals who strive to improve the quality of life for individuals with learning disabilities.

As a Clinical Lead, you will work closely with your multidisciplinary team (MDT) colleagues to support a defined caseload of adults at risk of placement breakdown. You will utilise your core specialist nursing skills to implement Positive Behavioural Support and coordinate assessment and interventions throughout the clinical pathway.

The post requires strong interface with various teams and services to promote shared formulation, including assessment and management of risks that may lead to exclusion, placement breakdown, or admission to inpatient services.

About the Role
  • Provide person-centred assessments to advise caregivers and junior staff on implementing and monitoring clinical interventions based on MDT clinical formulation.
  • Collaborate with Specialist Community Learning Disability Teams, in-patient services, and statutory/independent sector provider services.
  • Undertake specialist clinical training and updates, regular supervision, and professional leadership.
