Job Summary

We are looking for a motivated and experienced Clinical Nurse Practitioner to join our Community Learning Disability Intensive Support Team (IST) in South Staffordshire. As a Band 6 Clinical Nurse Practitioner, you will be responsible for providing high-quality, person-centred interventions to adults with a learning disability.

You will work collaboratively with the Specialist Community Learning Disability Teams, in-patient services, and statutory / independent sector provider services to deliver effective care and support to individuals with complex needs.

Key Responsibilities
  • Lead a defined caseload of adults who may be at risk of placement breakdown, including individuals in transition from children's services, specialist inpatient services or moving between community placements.
  • Provide person centred assessment and advise care givers and junior staff on implementing and monitoring clinical interventions based on MDT clinical formulation.
  • Work closely with your IST MDT colleagues to support adults with complex health care needs, such as Behaviour that Challenges and Mental Health needs.
