Senior Residential Care Coordinator

We are currently seeking a highly skilled and experienced Senior Residential Care Coordinator to join our team at HC One. As a key member of our care leadership team, you will be responsible for coordinating and delivering high-quality care services in our care homes. This is a fantastic opportunity to utilize your expertise in care coordination and lead a team of care professionals in providing exceptional care to our residents.
Your primary responsibilities will include managing care teams, developing and implementing care plans, and ensuring compliance with regulatory requirements. Strong communication and interpersonal skills are essential for this role, as you will need to collaborate effectively with multidisciplinary teams, including healthcare professionals, family members, and other stakeholders.
A strong background in care coordination and experience working in residential care settings are prerequisites for this position. We offer a competitive salary package, including £28,000 - £32,500 per annum, plus benefits such as paid Enhanced DBS/PVG, free uniform, company pension scheme, and more.
