About the Role

We are seeking a highly skilled and experienced Nurse Lead Care Coordinator to join our team at Conexus Healthcare CIC. As a key member of our clinical team, you will play a vital role in delivering high-quality patient care and supporting our healthcare professionals in providing exceptional services to our patients.

Key Responsibilities
  • Lead the development and implementation of care coordination strategies to improve patient outcomes and reduce hospital admissions.
  • Collaborate with healthcare professionals to identify and address the needs of housebound and hard-to-reach patients.
  • Provide clinical leadership and guidance to care coordinators and other healthcare professionals to ensure the delivery of high-quality patient care.
  • Develop and maintain effective relationships with patients, families, and carers to ensure their needs are met and they receive the best possible care.
  • Work closely with our primary care network to identify opportunities for improvement and implement changes to enhance patient care.
  • Contribute to the development and implementation of population health strategies to improve patient outcomes and reduce health inequalities.
Requirements
  • Qualified Registered General Nurse or equivalent with current NMC registration.
  • Substantial post-registration experience in a primary care or community setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team, with a strong focus on patient-centered care.
  • Knowledge of clinical governance and audit, with the ability to apply this knowledge in a practical setting.
  • IT skills in Microsoft applications, with the ability to learn new systems and technologies.
