Job Summary

We are seeking a highly skilled and experienced Nurse Lead Care Coordinator to join our team at Conexus Healthcare CIC. As a key member of our Primary Care Network (PCN), you will play a vital role in delivering high-quality patient care and improving health outcomes.

About the Role

The successful candidate will be responsible for providing clinical leadership and coordination of care for patients who are housebound or hard to reach. You will work closely with our existing non-clinical care coordinators and other healthcare professionals to ensure seamless care delivery.

Key Responsibilities
  • Provide clinical leadership and guidance to the care coordination team
  • Develop and implement effective care plans for patients who are housebound or hard to reach
  • Work closely with patients, families, and carers to ensure their needs are met
  • Collaborate with other healthcare professionals to ensure comprehensive care delivery
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed
Requirements

To be successful in this role, you will need:

  • A full UK driving license and ability to travel across the PCN footprint
  • Substantial post-registration experience in a primary care setting
  • Practical experience of clinical procedures and wound management
  • Experience of working with housebound patients and hard to reach groups
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
