About the RoleThe successful candidate will be responsible for leading a team of care coordinators and working closely with our GP surgeries to provide proactive and patient-centered care. You will be expected to:
- Identify and visit patients who are housebound or hard to reach, providing personalized care and support.
- Lead the roving multi-disciplinary care coordination team, ensuring seamless communication and collaboration with other healthcare professionals.
- Assess patient needs and coordinate the most appropriate care, working closely with care coordinators, nursing associates, and other PCN staff.
- Prioritize health problems and intervene appropriately to assist patients in complex, urgent, or emergency situations.
- Utilize population health intelligence to proactively work with patients, delivering personalized care and support.
- Support patients in utilizing decision aids for shared decision-making conversations.
- Work with non-clinical care coordinators to develop personalized care and support plans (PCSPs), ensuring holistic and patient-centered care.
- Support practices with nursing care as needed.
- Identify and work towards PCN and practice targets.
Conexus Healthcare CIC is a leading healthcare provider, serving a patient population of approximately 64,000 across five GP surgeries in the Wakefield district. RequirementsTo be successful in this role, you will need:
- A full UK driving license with relevant car insurance or the ability to travel independently across the PCN footprint.
- Substantial post-registration experience, preferably in a primary care environment.
- Practical experience of clinical procedures, including wound management and working with housebound patients.
- Experience of working in a multidisciplinary team (MDT) setting.
- Excellent communication and interpersonal skills, with the ability to motivate and empower others.
- Ability to work autonomously, managing your own caseload and prioritizing tasks effectively.
