Nurse Lead Care Coordinator

We are seeking a highly skilled and experienced Nurse Lead Care Coordinator to join our team at Conexus Healthcare CIC. As a key member of our clinical team, you will play a vital role in delivering high-quality patient care and supporting our healthcare professionals in providing exceptional services to our patients.
Key Responsibilities- Lead the development and implementation of care coordination strategies to improve patient outcomes and reduce hospital admissions.
- Collaborate with healthcare professionals to identify and address the needs of housebound and hard-to-reach patients.
- Provide clinical leadership and guidance to care coordinators and other healthcare professionals to ensure the delivery of high-quality patient care.
- Develop and maintain effective relationships with patients, families, and carers to ensure their needs are met and they receive the best possible care.
- Work closely with our healthcare teams to identify and address any gaps in care and develop strategies to improve patient outcomes.
- Contribute to the development and implementation of policies and procedures to ensure the delivery of high-quality patient care.
- Participate in quality improvement initiatives to ensure the delivery of high-quality patient care and improve patient outcomes.
- Qualified Registered General Nurse or equivalent with current NMC registration.
- Substantial post-registration experience in a primary care environment.
- Practical experience of clinical procedures and wound management.
- Experience of working in a multidisciplinary team setting and documenting and recording clinical information.
- Good standard of general education and good standard of spoken and written English language.
- Evidence of continuing professional development within the last 12 months.
