Job Title: PCN Nurse Lead Care Co-ordinator

We are seeking a highly skilled and experienced Nurse Lead Care Co-ordinator to join our team at Conexus Healthcare CIC. As a key member of our Primary Care Network (PCN), you will play a vital role in delivering high-quality patient care and improving health outcomes.

Job Summary:

The successful candidate will be responsible for providing clinical leadership and coordination of care for patients who are housebound or hard to reach. You will work closely with our existing non-nurse Care Co-ordinators and other healthcare professionals to ensure seamless care delivery.

Main Responsibilities:
  • Provide clinical leadership and guidance to the Care Co-ordination team
  • Identify and visit patients who are housebound or hard to reach, and develop personalized care plans
  • Work collaboratively with other healthcare professionals to ensure effective care delivery
  • Monitor and review patient progress, and make adjustments to care plans as needed
  • Develop and maintain strong relationships with patients, families, and carers
  • Contribute to the development and implementation of care pathways and protocols
  • Participate in quality improvement initiatives and contribute to the improvement of patient care
