Job Summary

We are seeking a highly skilled and experienced Nurse Lead Care Coordinator to join our team at Conexus Healthcare CIC. As a key member of our Primary Care Network (PCN), you will play a vital role in delivering high-quality patient care and improving health outcomes.

About the Role

The successful candidate will be responsible for leading a team of care coordinators, providing clinical guidance and support to ensure the delivery of excellent patient care. You will work closely with our existing clinical teams, including social prescribers, care coordinators, and health and wellbeing teams, to proactively identify and address the needs of housebound and hard-to-reach patients.

Key responsibilities will include:

  • Providing and maintaining a high standard of nursing and care coordination for patients who are housebound or deemed hard to reach.
  • Reducing unplanned hospital admissions by supporting patients to be healthy, monitoring their long-term conditions, promoting screening and health prevention.
  • Visiting patients who would benefit from outreach by providing a long-term condition service.
  • Engaging with patients' family members and/or carers to ensure they are supported to access services, promoting referrals to the care coordinators within the team.
  • Providing nurse leadership as required to the existing developed team, including training needs and support where required.
  • Promoting and engaging with vaccination campaigns, including Flu and Covid.
  • Engaging with community events and the wider teams, including both secondary and vocational care.
Requirements

To be successful in this role, you will need:

  • A full UK driving licence with the relevant car insurance to cover travel for business purposes or the ability to be independently mobile and travel across the footprint of PCN South on a regular basis.
  • Ability to work autonomously, managing your own caseload.
  • Knowledge of clinical governance and audit, as well as relevant professional and clinical legislation.
  • IT skills in Microsoft applications.
  • Effective communication and interpersonal skills.
  • Ability to work well as a team member.
  • Initiative and potential ability to work without direct supervision, problem-solve, and prioritise your own work.
Desirable Qualifications and Experience

We are looking for candidates with:

  • Substantial post-registration experience.
  • Practical experience of clinical procedures as per job profile/advertised requirements.
  • Experience of working in a primary care environment.
  • Experience of working with housebound patients and hard-to-reach groups.
  • Experience of wound management.
  • Experience of working in an MDT setting.
  • Experience of documenting and record-keeping of clinical information.
