Job Summary

We are seeking a skilled and experienced Nurse Lead Care Coordinator to join our team at Conexus Healthcare CIC. As a key member of our Primary Care Network (PCN), you will play a vital role in delivering high-quality patient care and improving health outcomes.

Main Responsibilities

  • Provide expert nursing care and coordination to patients who are housebound or hard to reach, ensuring they receive the support they need to manage their long-term conditions.
  • Work closely with our non-clinical care coordinators to develop and implement personalized care plans that meet the unique needs of each patient.
  • Lead the roving multi-disciplinary care coordination team, supporting team members to deliver excellent patient care and achieve our PCN targets.
  • Assess patient needs, coordinate care, and intervene in emergency situations, ensuring timely and effective support.
  • Utilize population health intelligence to proactively work with patients, delivering personalized care and promoting health prevention.
  • Support patients in making informed decisions about their care, using decision aids and shared decision-making conversations.
  • Develop and implement care plans that address the holistic needs of patients, working with our non-clinical care coordinators to create personalized care and support plans (PCSPs).
  • Collaborate with our clinical teams to deliver excellent patient care, supporting practices with nursing care as needed.
  • Contribute to the development and achievement of PCN and practice targets, ensuring we deliver high-quality care and improve health outcomes.

Requirements

  • Qualified Registered General Nurse or equivalent with current NMC registration.
  • Substantial post-registration experience, with a strong background in clinical procedures and patient care.
  • Experience of working in a primary care environment, with a focus on housebound patients and hard-to-reach groups.
  • Excellent communication and interpersonal skills, with the ability to work autonomously and as part of a team.
  • Ability to motivate, empower, and enable others, with a strong commitment to achieving better outcomes for patients.
