PCN Nurse Lead Care Coordinator

We are seeking a highly skilled and experienced Lead Nurse Clinical Care Coordinator to join our team at Conexus Healthcare CIC. As a key member of our Primary Care Network (PCN), you will play a vital role in delivering high-quality patient care and improving health outcomes.
Key Responsibilities- Lead the development and implementation of care coordination strategies to improve patient outcomes and reduce hospital admissions.
- Work closely with our clinical teams, including GPs, nurses, and other healthcare professionals, to ensure seamless care coordination and patient-centered care.
- Provide expert clinical guidance and support to our care coordinators and other team members to ensure the highest standards of patient care.
- Develop and maintain strong relationships with our patients, their families, and carers to ensure their needs are met and they receive the best possible care.
- Contribute to the development and implementation of our PCN's strategic plans and objectives, ensuring alignment with our overall mission and vision.
- Collaborate with our community partners, including social prescribers, health and wellbeing teams, and other stakeholders, to ensure comprehensive and coordinated care.
- Stay up-to-date with the latest clinical guidelines, research, and best practices to ensure our care coordination services are evidence-based and effective.
- Qualified Registered General Nurse or equivalent with current NMC registration.
- Substantial post-registration experience in a primary care or community setting.
- Proven experience in care coordination, clinical leadership, and team management.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work autonomously and as part of a team, with a strong focus on patient-centered care.
- Knowledge of clinical governance, audit, and quality improvement principles.
- IT skills in Microsoft applications, including Excel, Word, and PowerPoint.
