Nurse Lead Care Coordinator

We are seeking a highly skilled and experienced Nurse Lead Care Co-ordinator to join our team at Conexus Healthcare CIC. As a key member of our Primary Care Network (PCN), you will play a vital role in delivering high-quality patient care and improving health outcomes.
About the RoleThe successful candidate will be responsible for leading a team of care co-ordinators and working closely with our GP surgeries to provide proactive and holistic care to patients who are housebound or hard to reach. You will be expected to:
- Identify and visit patients who require additional support, working closely with our care co-ordinators and other healthcare professionals to develop personalized care plans.
- Lead the roving multi-disciplinary care co-ordination team, ensuring seamless communication and collaboration between team members.
- Assess patient needs and coordinate the most appropriate care, including referrals to specialist services as needed.
- Prioritize health problems and intervene appropriately to assist patients in complex, urgent, or emergency situations.
- Utilize population health intelligence to proactively work with patients to deliver personalized care, alongside our non-clinical care co-ordinators.
- Support patients to utilize decision aids in preparation for shared decision-making conversations.
- Work with our non-clinical care co-ordinators to develop and implement personalized care and support plans (PCSPs) that meet the unique needs of each patient.
