We are seeking a skilled and experienced Nurse Lead Care Coordinator to join our team at Conexus Healthcare CIC. As a key member of our Primary Care Network (PCN), you will play a vital role in delivering high-quality patient care and improving health outcomes.
Main Responsibilities
- Provide expert nursing care and coordination to patients who are housebound or hard to reach, ensuring they receive the support they need to manage their long-term conditions.
- Work closely with our non-clinical care coordinators to develop and implement personalized care plans that meet the unique needs of each patient.
- Collaborate with our multidisciplinary team to identify and address health inequalities, promoting health and wellbeing across our community.
- Lead and support the development of our care coordination team, providing training and guidance to ensure they have the skills and knowledge needed to deliver exceptional patient care.
- Contribute to the development and implementation of our PCN's strategic plans, working closely with our leadership team to drive improvement and innovation.

Requirements

- Ability to work autonomously, managing your own caseload and prioritizing tasks to meet deadlines.
- Strong leadership and coaching skills, with the ability to motivate and empower others to deliver exceptional patient care.
