Mental Health Practitioner

We are seeking an experienced mental health practitioner to join our community team. As a Band 6/5 practitioner, you will work with patients over the age of 18, focusing on OPMH patients with either an acute mental health concern or worsening of a pre-existing condition.
Key responsibilities include conducting full, holistic, biopsychosocial mental health assessments, completing recovery-focused care plans with patients, and working collaboratively with our acute physical health colleagues in Queen Alexandra Hospital to deliver holistic integrated care.
This is an excellent opportunity for experienced practitioners seeking to progress their career within liaison. Applicants will be supported to gain competence through extensive in-house training and bespoke supervision.
