Job Summary

We are seeking a highly skilled Physiotherapist to join our team in Stockport NHS Foundation Trust. As a Physiotherapist in our Intermediate Care at Home team, you will play a vital role in providing person-centered care to patients in their own homes or community settings.

Main Responsibilities
  • Work as part of a multi-disciplinary team to assess and treat patients with a range of conditions, including those with complex needs and mobility issues.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and effective discharge planning.
  • Provide education and support to patients and their families to promote independence and self-management.
  • Contribute to the development and implementation of clinical guidelines and policies to improve patient outcomes.

Person Specification

We are looking for a Physiotherapist who:

  • Has a degree in Physiotherapy and is registered with the Health and Care Professions Council.
  • Has experience of working in a community or intermediate care setting.
  • Has excellent communication and interpersonal skills.
  • Is able to work independently and as part of a team.
  • Has a strong commitment to patient-centered care and continuous professional development.
