About the Role

We are seeking a highly skilled and motivated Physiotherapist to join our Intermediate Care at Home team at Stockport NHS Foundation Trust. As a Physiotherapist, you will play a vital role in providing high-quality care to patients in their own homes, helping them to regain independence and improve their overall health and wellbeing.

Key Responsibilities
  • Assess patients in their own homes to identify their physical and functional needs
  • Develop and implement individualized care plans to meet the needs of patients
  • Work collaboratively with other healthcare professionals to provide comprehensive care
  • Conduct regular reviews and assessments to ensure patients are meeting their care goals
Requirements
  • HCPC registration as a Physiotherapist
  • Experience of working in a community setting
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
