About the Role

We are seeking a highly skilled Physiotherapist to join our Discharge to Assess team at Stockport NHS Foundation Trust. As a key member of our multidisciplinary team, you will work closely with patients, families, and healthcare professionals to provide person-centered care and support.

Key Responsibilities
  • Assess patients in their own homes or in designated residential care homes to identify their rehabilitation needs.
  • Develop and implement individualized care plans to promote independence, mobility, and overall well-being.
  • Collaborate with other healthcare professionals to ensure seamless transitions and effective discharge planning.
  • Provide education and support to patients and their families to promote self-management and empowerment.
  • Contribute to the development and implementation of clinical guidelines and policies to improve patient outcomes.
Requirements
  • HCPC registration as a Physiotherapist.
  • Experience of working in a community or intermediate care setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work across 7 days, including evenings and weekends.
