We are seeking a qualified Physiotherapist to join our team in Stockport NHS Foundation Trust's Intermediate Care at Home and Discharge to Assess service. As a Physiotherapist, you will work within a multi-professional team to provide person-centred care to patients in their own homes or in designated residential care services.
Key Responsibilities- Assess patients' needs and develop individualised care plans to support their recovery and independence.
- Work collaboratively with other healthcare professionals to provide holistic, multidisciplinary interventions and support.
- Conduct home visits and assessments to identify patients' needs and develop care plans.
- Provide education and support to patients and their families to promote self-management and independence.
- Participate in complex discharge planning to ensure smooth transitions between care settings.
- HCPC registration as a Physiotherapist.
- Experience of working in a community setting and with patients with complex needs.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
