About the Role

We are seeking a qualified Physiotherapist to join our Intermediate Care at Home team, providing person-centred care to patients in their own homes. As a key member of our multidisciplinary team, you will work closely with patients, families, and other healthcare professionals to deliver high-quality care and support.

Key Responsibilities

  • Assess and develop individualised care plans for patients in their own homes
  • Deliver physiotherapy interventions to patients with a range of conditions, including those with complex needs
  • Work collaboratively with other healthcare professionals to ensure seamless care transitions and effective discharge planning
  • Provide education and support to patients and their families to promote independence and self-management

Requirements

  • HCPC registration as a Physiotherapist
  • Experience of working in a community or intermediate care setting
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
